* Diameter of any Hail (in inches or relate to coin size)
* Intensity of the Rainfall
(Steady Rain 1/2 inch or more in 1/2 hour, very heavy / cloudburst)
* Estimate any Wind (speed and direction)
* Damage (Seen or heard about in your area)
* Injuries / Deaths

Funnel Clouds / Tornadoes:

* What You See
* Where
* Speed and Direction of Movement
* When Observed
* Damage (Seen or heard about in your area)
* Injuries / Deaths

Flash Flooding / Flooding:

* Time Flooding Began
* Name and Location of Stream / Creek
* Damage (Seen or heard about in your area)
* Injuries / Deaths
* Rainfall Rates

Winter Storm / Snow / Blizzard:

* Condition of Roads in Area
* Visibility Through Falling / Blowing / Drifting Snow

Fog / Mist:
